More about the annual High Risk Data Attestation

What is it?

The attestation is an online course that records if you work with, or could reasonably be exposed to, protected and regulated data, including protected health information (PHI), personally identifiable information (PII), and other confidential data. These data are termed high risk because losing the data could cause significant harm to the subject of the data, and to yourself and the institution if we fail to protect it adequately.

When you take the attestation, you will be asked to provide an inventory of the devices you use to store and access WCM high risk data – like laptops, phones, and tablets. From there, you will respond to a series of statements about safeguarding this data in compliance with WCM security and privacy policies.

Is there a deadline?

Once you receive the link to the course in your inbox, you have 45 days to complete it.

I submitted my attestation but I want to go back and change something. Is that possible?

No, once the attestation has been submitted it cannot be reopened or reset.

I have a question or a concern

If you have any questions about the course or your exposure to high risk data at WCM, or if you experience technical issues accessing the attestation please email support@med.cornell.edu.
